Crash on iOS when disconnecting (#93)

Application based on Halcyon crashes on disconnecting inside RequestsManager:: timeoutAll() method. Most likely the issue is caused by concurrent manipulation of requests Map.

After implementing lock/synchronization on requests map, the issue seams to be resolved.
